that can be downloaded and filled in
on your PC. This questionnaire
covers Ceremony Music, Dinner
Music, Entrance songs, Special
Dance songs and introductions.

Ceremony & Reception - If
equipment does not need to be

|Kids today are so in tune with all the
latest music and dances and so am I! I
offer radio friendly versions of all of
today's top hits and know how to work
with the kids to get them dancing and
staying that way the entire party. Again I
know schools have limited finances when
it comes to hiring a DJ, so please call or
email so we can come up with a price
that works best for all of us.
|I 'm all about helping out those in need. I have a long history of playing for
charitable organizations. I also understand that these events are usually handled
with limited funds. Rather than list a quote for such an event, please contact me
directly so we can speak about your options for my services at a reduced price.
|Travel Rates - All above rates are based on a 30 mile radius of event from location
for "Music By D" (North Modesto ). An extra $50.00 will be added on for every
additional 30 miles of travel to the event. (unless otherwise negotiated)
Payments - There is a $100 non-refundable deposit to hold your date due at signing of
contract. Half of the remainder balance must be paid 45 days prior to event, again
non-refundable. The remaining balance must be paid no less than 2 weeks prior to the
event, again non-refundable.
Holiday Rate - There will be an additional charge of $200.00 for any event landing
on a Holiday or Holiday weekend.
